Istredd knows that the voices are a fantasy. They tried to deceive him with Yennefer’s voice first, which he logically knew was impossible, and he isn’t out of his mind like the rest of them. But he understands what it is trying to do, whatever is in there. The question is if it’s for the people who kidnapped them, or for this shrine and whatever god it represents. They painted a symbol over it, it wasn’t there before. It is possible they have no idea what it is.

“One of the reasons why you shouldn’t be wandering off alone. It could have been anyone else, including the things living in here with us.” Wilhelm would have struggled even with his magic, so by himself, he would be a lost cause. Something could snatch him before any of them could do anything. He keeps a hand on the boy and plans on leading him back until he sees the skin.

That is definitely concerning but at this point unsurprising, whatever the ritual did, it is more than your average psychedelic. It doesn’t make it any less concerning.
