"It's consistent with traumatic brain injury." Cas touches gingerly at the laceration at the back of his head. Regardless of what's going on internally, that needs to be healed to prevent infection alone. "Was your head struck during capture? From blunt force or a fall."

He frowns, breathes out slowly, and wills his Grace to the surface. He'd already forced it twice in while healing Wilhelm, now it's bringing pain, nausea, and dizziness to him as he drags the power out of himself. Palm settling over the wound at the back of Kell's head, a brilliant, white light seeps out between his fingers, bright enough in the dim cave Kell likely sees it. A pleasant, tingling sensation of warmth prickles over Kell's scalp, and the damaged tissue begins to fuse closed, but only just barely. Only enough to close it in, skin still tender and delicate. Cas tries to take some of the swelling with, but this small heal was already so costly. He gasps a breath when it's finished, swaying enough he has to steady himself on Kell's shoulder. But he shakes his head, brushes it off quickly as he can to carry on before there's a chance to comment on it.

"We should hope it's only a concussion." Castiel's bedside manner isn't fantastic, but intercranial pressure is a death sentence if significantly advanced. His pulse and breathing seem fine, but with Castiel's senses on the fritz, determining blood pressure is beyond him. The next few questions are rattled off in quick succession, half because of aforementioned shitty bedside manner, and half to test Kell's ability to follow along and respond appropriately to each. "Have you had trouble moving or speaking? Has anyone commented on changes in your mood or behavior? Any seizures?"

Each would be signs of dangerously advanced pressure. It could be due to anything from swelling around a contusion (manageable), or a much more concerning brain bleed or pooling fluid. The later has precious few treatment options where they are, and Cas is so low on Grace as it is.

"Don't tax your mind the next 48 hours, keep as still and rested as you're able. The harder your brain has to work, the worse your injury will become. Keep your head elevated above your heart, especially when sleeping. Alert me the moment your headache becomes more severe, you begin convulsing, or lose function to one side of your body - do you understand?" Spoken firmly, because Cas will not see this boy die, and maybe a little bit because he refuses to admit failure to Lucifer of all people.
